See pg. 2-23 of the MDS manual which states, “When residents are in a hospital outpatient department for an observational stay of less than 24 hours and the resident is not admitted for acute care as an inpatient” then a d/c tracking is not done.
Also, see pg 2-38 which states “The ard is not altered if the beneficiary is out of the facility for a temporary leave of absence during part of an observation period. In this case, the facility may include services furnished during the beneficiary’s temporary leave of absence (when permitted under coding guidelines- see Chapter 3) but may not extend the observation period.”
I think that this will cover what you need. You will only need to extend your medicare benefits by 1 day since the resident was not in the building at midnight.

We have patient 10/13/03 under Medicare Part A, went to ER 10/25/03 at 11Pm, she came back to our building at 1am on 10/26, she is not admitted at the Hospital, just stayed at the ER. Our Billing Dept. wants me to discharge her in the system on 10/25 and do another assessment. I know we had discussed this before that if this happens, you just need to skip that day which is 10/25/03 and we dont get payed for room and board and continue and adjust the MDS calendar....Pls. help.... any documentation that you can provide to support my claim will be appreciated... and any suggestions what she needs to do?
