You don’t do a reentry after a type “8” (discharged prior to completion of initial assessment) or with a type “6” return not anticipated.  You will need to complete an admission assessment for both of these occurrences.  See the chart on pg. 2-26 of the MDS manual for further guidance.   Brenda W.
