A surgical wound is classified as an open or closed incision caused by surgery that is healing or not healing, they are considered healed after removal of sutures or staples. Skin grafts, drain tubes and sites of drain tubes that aren't healed yet can also be considered surgical wounds.  If the patient has an incision that isn't healed yet regardless of the type of surgery it should be coded as a surgical wound.
