When a resident comes into our facility with any type of incont. we initiate a bowel and bladder assessment - the assessment includes an assessment by the nurse, an interview with the resident and a 3-day pattern assessment. Upon completion of these portions and collection of all the data - the type of incont. is identified (i.e. functional, retention-overflow, neurogenic, stress etc...) and a program is developed based on the type of incont. identified. The majority (but not all) of our residents have functional incont. secondary to dementia or immobilityand the plan for functional incont. is usually habit training with a toileting schedule (based on the 3-day pattern assessment history) developed (along with other interventions to improve continence status). The type of incont. must be identified before a plan can be set. Holly QI in CT
