This is the first time this has happened and I do
not know what to do besides good documentation that I am aware the RAP should
have triggered and whether I will be careplanning.
Psychotropic RAP didn't trigger on a resident who
is on Risperdal and Zoloft. (Is on a skilled care Alzheimer unit)Section O is
marked accordingly (my area)
Section B has both short and long term memory
loss, no recall, poor decision making skills. B5abc and f marked as
"1"
Section E wanders and not easily
altered
What do I do?
Any help appreciated
