I thought the title of the assessment was, "Minimum Data Set". What did I miss?  On the other hand, I think what we are discussing is what I call a shotgun approach to documentation.  Just fill out all the 'forms' and perhaps we will hit the assessments we actually need.

If you read the MDS manual page 4-1 paragraph 4.1 it explains that the MDS alone does not provide a comprehensive assessment.  It is a preliminary screening.  .....................The triggers target conditions for
additional assessment and review, as warranted by MDS item responses..............

I disagree with having a
standard SET of ancillary assessment forms that are completed on all residents.  I recommend using additional ancillary assessments on triggered RAPs and any other condition, not triggered, that is essential to the resident's care and treatment.  A condition that may not trigger but may require additional assessment is the reason for a pacemaker.

Be very careful with getting rid of all your "assessments" that you do besides the MDS. Many years ago this happened in CT - many Nursing homes were told by so called "experts" in the field that the MDS replaced all those extra assessments we were doing, so they stopped doing them. The State came along and said - the MDS is just the "minimum" and the expectation in CT is that comprehensive assessments still need to be completed. Many homes were cited and since then most of us have gone back to doing all those x-tra assessments. States vary with this interpretation - call them and get their interpretation before you make any changes - many of the "speakers" out there are using generalizations or are speaking from only a handful of state interpretations.
