Bruises & Skin Tears, Incident Reports

We are not sure if we are required to do an incident report for every bruise and/or skin tear, or just those that we don't know the origin of, ie if we know a bruise was from an ankle being bumped during transfer, do we do an incident report?  Some residents bruise so easily, and bruise from every lab draw and frequent insulin shots.  Your advice? Thanks.
