You must have a 3 three night stay after 12/1- example if you where in the hospital 11/30 to 12/2 you would not qualify. If you where in the hospital 11/30 to 12/4 you would qualify
 
This can be found in the new CMS Coverage Manual- no change CMS site is down this evening so I can not get you the exact citation

completed 100 MCR days on 10/3/03. She went to hosp. 11/14/03 and readmit
back to our facility 11/17/03. My question is, is it before she reach 60
days or after 60 days.
