My opinion is that the answer to this is partially in the RAI manual page 4-1 paragraph 4.1. "The MDS alone does not provide a comprehensive assessments. Rather the MDS is used for preliminary screening to identify potential resident problems, strengths, and preferences. The RAPs are problem-oriented frameworks for ADDITIONAL assessment based on problem identification items (triggered conditions). ............. The Triggers target conditions for additional assessment..........."
See also page 4-18 When is the RAI Not Enough? This is only one page but too long to type here. These area in the manual should answer your question. No one can tell you that any specific documentation or 'form' is REQUIRED. It will depend on the results of the comprehensive assessment.
I dealt with this question last week, the MDS coordinator told me that she did not have time to read the manual, I was the consultant and I should just "tell her what to do."
I almost did,
but the client has been with me for 24 years so......I just smiled my best 'consultant' smile.

There was a recent thread on the listserve regarding all of the assessments that we do along with the MDS i.e. risk for pressure ulcer, side rail assessment, oral assessment, pain assessment, fall risk and on and on and on...
As usual, I never thought I would need the end result of all of this info and casually reviewed it then dumped it. Well, now of course we are reviewing the need for the assessments and trying to enter the assessments that we feel we need to keep into our software then train on their use.
My question is, what is actually required? We are not JCAHO certified.
The bottom line is if the MDS is a stand alone document, are the "assessment forms" really necessary?
