I've doing RAPS since "the beginning" and struggled for several years before
I became proficent with them. The longer you work with them, the easier
they'll become. Always keep the RAI manuel close at hand. Look at the first
section for each triggered RAP. It will tell you the INTENT of the RAP, as
well as WHAT triggered it. Try to address each area under GUIDELINE section
in some way. What I aim for is to have a RAP that is concise, but to the
point..... and also would tell the surveyor quite a bit about each resident
without having to look at the whole chart, and that staff familiar with the
residents could tell who I was talking about even if there was no name on
the RAP.
