You didn't mention what state you are in and the Other State required assessment is driven by when that particular state requires it to be done.  Depends on what your state requires as far as the OSRA is concerned.  In Indiana, the OSRA requirement is based on the State (Medicaid) RUG, not the Medicare RUG...so in your scenario in Indiana, yes you would do the end of therapy OSRA.  Can't speak for other states.
