I believe I would go with the 411.1, Intermediate Coronary Syndrome.

I am in LTC. The resident is one of our own who went to the hospital and returned with the DX of Acute Coronary Syndrome. One of the hospital nurses was calling it an acute MI, but the doc didn't write that on the orders.

There is a 411 code ( I believe it is 411.1) for something like "Intermediate Coronary Syndrome".  What setting are you in, and is the acute episode resolved? It sounds like something that would  be coded for the acute hospital stay.  Do you have other cardiac diagnoses which are more relevant to your setting?
