Hello, all:

I have developed total brain drain today and can't figure out how to get the
CMS forms I need. I can access the Roster Sample Matrix (802) and the
Resident Census and Conditions (672) already completed, but my DON wants
some blank forms. We are getting ready for that fun time known as SURVEY. We
have been deficiency free for the last 2 years, so we expect they will be
working extra hard to find something to ding us for this year.

Thanks for all your help. As usual, I know I can count on someone out there
to come to my rescue.
