I usually come in to work about 8 am.  This must have gone through sometimes during the night.  I think our server was not running right yesterday which delayed my emails.

Just wanted to say thanks to someone out there on this discussion group that submitted the basis of this form.  I took a form labeled nurses’ notes and expounded to make it mine for my NAC notes.  I do these with 5 day or 14 day, adm, annual, significant change.  I then use the update areas to write any changes that occur since the last assessment.  In the additional comments line, I write (if the resident is on med a) what they are being skilled for.  I used to write a long-hand note, but this has really saved me time and effort.  I copy my updates to the back of the form which takes up less chart space. 

 

Could whoever sent out the basis of this form in nurses’ notes, please email me privately at [EMAIL PROTECTED] so I can say thanks.
