Dena,
This will really depend on which type of cancer. I find that a general care plan  for a dx such as cancer is really difficult to write.  Instead, focus on what problem or need you have in relation to that diagnosis.  Is it risk for recurrence? Then your approaches should focus on monitoring/screening for s/sx of recurrence, and preventive measures, nutritional interventions, medications.
 
Is it pain/comfort?  Then a pain care plan will do nicely, with mention of the dx as a "related to" or "secondary to". 
 
Is the dx progressing or terminal?  Focus on death/dying issues and/or hospice referral as appropriate.
 
In other words, don't care plan for the diagnosis so much as for the individual needs associated with the diagnosis.
