she had done, without the exe extension. They are really good, and I
asked her permission to post them on the website. I wanted to go under
the MDS section in the Document Library and put in samples of Narrative RAPs as
well as these templates, if that's cool with you.

The
attachment "raps.exe" is most likely blocked by most users email
software and by many ISPs. Any document with an exe extension is an
"executable
program" which can contain viruses, Trojan horse, spyware or other more
dangerous programs.
I
strongly advise against posting anything with an "exe" extension and
discourage anyone from launching any email attachment with that extension.

I didn't give you a good explanation
of the form when I sent it. You'll notice that most of the front page of
each RAP is basically just copied out of the RAP KEY for each RAP.
On the back, you'll notice it says
"Nature of the Problem", and you should explain the problem, with all
the specifics for that resident.
"Complications and Risk
Factors" is the next level. In there, I put everything I'm worried
about. For example, for vision, in a hypertensive diabetic, you need to
keep watching them for early cataract development, hypertensive retinopathy,
and diabetic retinopathy. Also, we tend to need extra light to focus as
we get older, and we used extra lighting in the rooms in general, so I usually
stuck that in there as well. These folks are really susceptible to ocular
bleeds, so they really do need to be seen q. yr, or q. six months (depending on
the eye doc and degree of disease) so I usually will put "Needs to be seen
q. yr, per Dr. X" in the referral section, then make a copy, put the date
of the last exam in red, and send it to the social worker (or who ever makes
your appointments), to be certain it gets done. I mention hypertensive
control to maintain low B/P so as to protect both against higher risk for
retinopathy and bleeds. (Also, our last surveyor was a diabetic with
retinopathy, and she was on a rant on the subject!)
Next is "Need for
Referrals" and you would put in whatever would be appropriate, or indicate
why you aren't recommending them. For example: "Mrs. Jones was
seen by Physical Therapy on last RAP review for improvement in gait, but showed
no progress due to advanced cog. loss, and will not be referred again due to no
change in cog. level"
The last analysis subject is factors
considered in developing the Care Plan, and basically you just list everything
you checked under "guidelines" on the other side, as you should have
already discussed them in the analysis sections above on the back.

I'm like you, I type much faster
than I write. I did my own set of RAPs for all the nursing stuff. I
basically just copied the stuff off the Keys, and then put in sections for the
nature of the problem, the complications and risk factors, and need for
referrals, if any and then the factors considered to develop the care plan. I'm
including a sample Vision RAP. Since I did them in Word, you will have to
either have Word on Word Perfect 7 or above on your computer to use them.
You will also have to remember to hit the INSERT key in the word processor
while you do them or else the format gets seriously off, and items get pushed
off the page.
If you can use it where you work, I
can send you the set of nursing RAPs. Just remember, I didn't do any for
Psychosocial, Mood or Behavioral RAPs.
