Couldn't download your survey but in the facility I just left we
*    Measured hydration status of all resident by quarterly, or if necessary weekly urine specific gravity by Chem 10 dipsticks.
*    Did strict Intake on only those residents on hydration IV's or Tube Feedings. Or on residents when ordered.
*    Did strict Output on residents when ordered.
*    Monitored fluid build up for diurising in cases like CHF with daily AM weights
*    Handled intake by putting all fluids on the meal and snack trays and having staff document amount taken.
 
I have found the routine Q shift I&O in an uncontrolled setting like the SNF to be hysterically laughable and/or criminally negligent as far as their accuracy goes.
