This is a tough one, but does happen fairly often. When I am doing ADL or continence training, I teach to start at the bottom (most independent) and work your way up until the answer fits. So, for this one, I would eliminate the coding of 1 because it does not fit that "bowel incontinent episodes occur less than once a week"--since there were 2 episodes. So I would code it as a "2" bowel incontinent episodes occur once a week. It does not fit that one perfectly either, but is a better fit than the coding of "1". I know that moving to the 2 is a significant change criteria though. The decision might be affected if the episodes were on the same day. Was the resident incontinent twice on the same day ,or was it different days in the first week?

I have a question that I am just trying to get a consensus of opinions about on H1a bowel coding.
If a resident had two episodes of bowel incontinence in week 1 and no (zero) episodes in week 2 would that be coded as 1,2 or 3.
So far I am getting a 1 because it did not happen at all the second week from those who care concerned of a payment audit and 2 from those who are concerned about a state audit.
Please include your reasoning behind your answer.
