I have attached a standard care plan that we use in our buildings. We have guidelines 
that we use for determining implementation of this standard care plan. Then we update 
the care plan by adding date of any falls and adding or adjusting interventions as 
needed. The survey team has learned they do not have to hunt or ask for a fall log, as 
the info is always in the same place for our residents. The care plan is in Word, and 
you can tweak it to meet your individual facility needs.
