We had one gentleman that did this.  We finally put a sign of an
outhouse on the bathroom door.  Worked sometimes.
Have you tried scheduling his toilet needs to try to avoid the need for
him to go look for a place?

Does the white toilet blend in with the white floor??  We have had
success with contrasting colored toilet seats.  Was he a rural farmer or
outdoor worker?? Is he reverting to his work days when he would void by
the trees or bushes??  We have also dealt with this by using wall decals
and artificial plants around the toilet to simulate the outdoors.  Use
the disposable incontinent pads or plastic around the base of the plants
instead of Spanish moss or fake dirt.
