Hi all, This week we received instructions and a new advance notice for termination for Medicare + choice residents. It is very different from the 10055 and has a 2 day notice window. It is form 10095. It notifies the bene of a new fast track appeal process available through the QIO. The form states you will get a decision no later than one day after you request an appeal. I am scheduled to attend a 2 hour web based inservice provided  by the NJ QIO in a few days. This form and program will be effective January 1,2004. You can go to  www.cms.hhs.gov/medicare/bni/ for more info. You may want to check that the appropriate person in your facility has this information. Jeanne
