Your resident sounds pretty stable.  He doesn't appear to require daily skilled services from your description.   Daily subq injections are not a skilling service unless combined with frequent physician order changes and visits.  Since he is having no order changes, is only monitored weekly for blood count and is fairly independent, I think it would be a stretch to continue covering him.

Would Procrit s.c. injections q day and weekly CBC checks be sufficient to keep a resident skilled ?   The resident has max'ed out with PT and OT, and is maintaining his baseline functional status , etc...  Spouse will not take him home secondary to ADLs (does require assist. with transfers which she cannot safely negotiate).  He locomotes in w/c independently, no cognitive issues, no med changes.   He will be receiving Procrit for "pancytopenia", cause still undetermined (?).  Any suggestions would be great!  Thanks again...
