Nancy, We use the ARD as the date on which the
observation period ends. Anything that happens after that ARD is not captured on
that MDS but, depending on what assessment you are completing, it may be on the
next one. Our written assessment is not just based on that information,
but any information gathered up until the date the note is
written. Assessment is an ongoing thing. We also update the rap ongoing,
although this is not required. You really cannot complete an assessment
until after the ARD.
Just remember that the MDS is only a tool to help
identify any concerns. From that you begin your indepth
assessment.
