Nancy, We use the ARD as the date on which the observation period ends. Anything that happens after that ARD is not captured on that MDS but, depending on what assessment you are completing, it may be on the next one.  Our written assessment is not just based on that information, but any information gathered up until the date the note is written. Assessment is an ongoing thing. We also update the rap ongoing, although this is not required. You really cannot complete an assessment until after the ARD.
Just remember that the MDS is only a tool to help identify any concerns. From that you begin your indepth assessment.

I have a question that I'd like to know how everyone is handling this? 
This situation has come up a few times and unless I'm missing something, I don't see how to resolve it.   There are times when the ARD needs to be changed to capture a better RUG for a Medicare resident.  But what do we do when the ARD has been changed and one of the disciplines has not written their assessment note yet, and now with the new ARD it will be outside of the window?   Am I missing something?  How does everyone handle this?   I would appreciate any info here.
