YES! Is therapy still seeing her? If not, do an ORMA but keep her skilled until Protimes are stable.

I was reviewing her chart to see if she is skilled nursing. she has had multiple protimes/inr's drawn. within the last week here is how her coumadin and protimes stand: 12/12 md ordered for coumadin 1 mg to start on 12/13/2003 with a protime/inr on 12/15/03. her 12/15 protime/inr was 23.0/2.2. Md kept coumadin at 1 mg with a protime on 12/19/2003. the 12/19/03 protime/inr came back elevated so md place coumadin on hold for 24 hours then we were to restart at 0.5mg qd with next protime on 12/22/2003. Protime was drawn this am results came back 49.7/4.9 awaiting return call from md. but I suspect he will hold coumadin and give us new orders for a protime. We have also withing the last week found a very large bruise on resident. Would you continue to skille her until her protimes are more stable.
