You go ahead and submit your 5 day/significant change mds.  When you resident comes back from the hospital, reassess to see if there has been a significant change. 

 

If not:  Restart your pps cycle by doing a readm/return (5 day) assessment.  Coded under AA8b as a 5.  Set up a care plan conference and formulate your care plan.

 

If so:  Restart your pps cycle as above.  Complete a significant change assessment with either the 5 day or 14 day assessment and then do your careplan within 7 days of signing Vb2 of the mds.

I had a resident who has been in and out of the hospital, medicare A.  We were doing a 5day/significant change mds.  However, we completed the mds but she went to the hospital before the careplan.   So what do I do?  Do I submit the 5day/significant change mds?  But then when she comes back from the hospital I know I do another 5day, but can I call that one a significant change too?  Should I change this current 5day/sig change mds to just an MPAF?   That would be extra work since it's already completed.  What do I do here?   She is in the hospital right now.  Her careplan was supposed to be today.  We have never held a careplan when the resident wasnt' in the building.
