You go ahead and submit your 5 day/significant change mds.  When you resident comes back from the hospital, reassess to see if there has been a significant change.    If not:  Restart your pps cycle by doing a readm/return (5 day) assessment.  Coded under AA8b as a 5.  Set up a care plan conference and formulate your care plan.   If so:  Restart your pps cycle as above.  Complete a significant change assessment with either the 5 day or 14 day assessment and then do your careplan within 7 days of signing Vb2 of the mds.    Brenda W.
