this may be a little tacky but I believe every MDS coordinator has their
moment of desperation. I am in need of the fallowing: 1.)Self-Medication
assessment/protocol/forms
2.)Functional
limitation/contracture forms(sec G # 3&4 how you get this info) Copy the test right out of the RAI manual using Microsoft
Word. Include the instructions for doing the test on your
form
3.)
ADL forms that are working for you!
4.)Care
Plan letters to families inviting them to attend care plan
meetings
