If you are talking about P2a "Special Behavior Symptom evaluation program", then I don't think that what your are describing is enough to code this question.  The manual on page 3-190 states "A program of ongoing comprehensive, interdisciplinary evaluation of behavioral symptoms (such as the symptoms described in Item E4).  The purpose of such a program is to attempt to understand the "meaning" behind the resident's behavioral symptoms in relation to the resident's health and functional status, and social and physical environment.  The ultimate goal of the evaluation is to develop and implement a plan of care that serves to reduce distressing symptoms."
Simply recording the presence or absence of behavior symptoms on a medication sheet doesn't seem to fulfill this definition.  I think that the program that they are looking for, is to have a way of determining the cause of the behavior.  A form that shows what was happening before the behavior occurred, the behavior, the intervention and the effects of the intervention for a short period of observation time followed by an interdisciplinary team meeting to discuss the causes of the behavior (time, activities, meds, etc) in order to determine a plan of care specifically for the problem behavior is what is necessary.

Could this be a behavioral Mgt Program when you have a Careplan to reduce behavioral symptoms and also using a behavior med sheet ? I have read the definition in the RAI manual but it  still confuses me.   Any help will be appreciated.
