Backdating is not acceptable.  You put the date you actually signed the
form.  If you wanted a more complete record, you could also write on the
form, "The care planning decisions were completed (last week's date), but
not signed until today."

Last week we held are care plan meeting for a resident and we forgot to sign
and date VB3 and VB4.  Our HIM brought the MDS to me today to sign and date
VB3 and VB4.  Here's my question:  what date do I put in VB4?  The RAI
Manual says that VB4 is "the date on which a staff member completes the care
planning decision column (VAb) which is done after the care plan is
completed".  In this situation, the care planning decision column was
completed last week, so do I put last week's date in VB4?
