I was wondering if anyone has a form they use for the disciplines to fill out for care conferences. We are a small hosp. based snu.Lots of turnover, most stays less than 14 days. We ( charge nurse, director rehab, case manager and mds.) meet every day for a medicare conf. discuss admits and discharges overall issues. But once a week we hold a conf. with all disciplines to discuss progress, issues , concerns etc. The residents and families are invited to attend but usually never do. We have a form we currently use but it is just divided into disciplines and they write a short blurb. I usually have a very hard time reading it, and lots of abreviations are used. We ( myself or case manager ) are suppose to go to each resident and up date them as to what we talked about, well as I said the form is user unfriendly and I am unable to relay what it says if I cannot understand it. I would like a form preferably a check list or multiple choice that would be easier to understand for the resident. I am trying to revamp ours but it will be time consuming and thought why reinvent one if their may be one out there that I can model mine after. You can e-mail it to me privately if you are able . The fax machine I use is a floor one and I'm afraid I would not receive it. If I knew when it was coming I could keep and eye out for it. Thanks for your help... Karen.
