Gerald - attached is a cost of care template that my firm (KPMG) developed.  We presented this at the Pennsylvania Association of Not-for-Profit Homes (PANPHA) conference in May 2003.  You can certainly use it - it needs to be customized for your facility.  Under Drugs, you need to identify with the assistance of your pharmacy the charge to your facility for certain specific (costly) Medicare Part A drugs - this could be a flat (capitated) rate, or a per drug cost.  Drugs you want to specifically cost out would be items such as erythropoetin injection, Vancomycin IV, TPN, possibly certain AIDS drugs.  The original intent of CMS was that facilities would have "winners" and "losers"  RUGS - some RUGS would make money, others lose money, it would all be beneficial in the end. 
 
You can obtain equipment costs from your CSR, purchasing agent, or whoever orders this stuff.  I would put down equipment only if you have to obtain it specially, such as rent it or lease it.  Bariatric beds and lifts can be especially costly.
