At our facility, we have initial orders
for “eval and tx”. When the therapist evaluates the patient
they complete a 700 form which states the long term and short term goals for
the resident along with the planned tx 5 x week for 30 minutes, etc….That
form goes to the MD for signature so wouldn’t that qualify for
clarification. Its signed by the MD and it is a written tx plan.
