This sounds like a facility or corporation policy. In one of my former facilities the new Adminstrator and DON decided that this would be our policy (after some med orders were missed on a few re-admits). At the time we had a resident who was D/C'd & Readmitted multiple times. However, once all the Dept Heads realized that they had to redo their inital assessments ands all the other paperwork that is involved in a "new" admit each & every time he came back from the hospital they/ we started complainig. Finally we were allowed to place copies of all the inital paperwork and previous MDSs/ careplans on chart after they had been stamped "COPY."

Please help me clarify what is regulation on medicare A residents. If medicare A goes out to the hospital I d/c return anticipated and upon return I do a sig. change assess./5day or if no change I do na/5day. I then review raps if indicated and pull forward if no changes. I leave 15 mos of assessments on the chart at all times regardless of admit or readmit. I have been told recently to completely d/c chart and start all over with new raps with each admit and that the 15 mos only apples to icf not medicare. Would someone please clarify this for me.
