My last message was undeliverable, I think our server at work malfunctioned so I am sending this reply again.
It has been out on the table about doing significant changes after the completion of therapy even if improvement is expected and care planned as such. The way I have looked at this, if the resident improves enough that 2 or more improvements in adl occurs, usually the care plan is impacted. I usually do a significant change for this reason: it will show a true picture of the resident once rehab has completed and they are functioning at maximum potential. I have had surveyors look for these significant changes and this has saved us during survey.
