Our (AR) State RAI Coordinator said if it
met any of the criteria ( unable to remove or restricts) it is to be coded as a
restraint.

Subject: siderail vs restraint?
For section P side rails is it not true that if the resident cannot put
the siderails down alone then it is considered a restraint? The
siderails on the beds at my facility I would not be able to put down if I were
in the bed therefore all siderails would be a restraint?

P4 is only for coding devices that serve as
restraints. If the siderail is not a restraint, do not code it in section
P.
RAI Manual Page 3-198 "The intent is to
evaluate as part of the assessment process whether or not a devices meets
the definition of physical restraint, and then code only those devices
categorized in section P4 that have the effect of restraining the
resident."

Subject: Section P
Please share how you code Section P Devices
and Restraints. If siderails are for enablers only, do you mark
siderails in that section at all? Do you code devices that are NOT
restraints (i.e. lap buddy which R can
remove)?
