wound care regs

Can anyone tell me what the skilled requirement for wound care on decubitus ulcer is?  Is this correct?
       1.  Wound care has to be done daily.
       2.  2 or more stage 2 ulcer or one stage 3 or 4

Is wet to dry dressing change considered skill if done daily?  Where can I find the regulation?
