We would need a little more information in order to
answer you question accurately. If your resident self-propels but is
requiring supervision the self-performance for that is 1 but it sounds as if
staff is also wheeling the resident to meals. When staff is pushing it is
4 , but could not code 4 because resident also self-propelled, so the most
accurate response would be extensive assistance. I am not clear what
happens when the resident goes off the unit. If the resident does not
wheel himself at all off unit, it could be total but if the resident does some
of the wheeling off unit, it would again be coded as extensive.

facility a short time, you could complete a 5 day PPS assessment and code
the information from the hospital which would place the resident in a RUGS

According to our
adm. if a resident leaves the facility say prior to 12mn on the 18th
the last actual day of service would be 17th due to the "midnight
rule". how about the res that is admitted on the 17th and leaves the
facility 2-3 hours later would you do a 5 day or a disch prior to
completion of assess and just take a default rate?
