I’m going to try to reply to this but I don’t completely understand what you are saying. Your care plan must follow the MDS. Per the manual that is not an option. With OBRA assessments, you have 7 days after the completion of R2b (quarterly, significant correction of quarterly) or VB2 (admission, annual, significant change, significant correction of a quarterly) to complete your care plan. You are right that the care plans must be updated every quarter and I too use quarterly dates for my goal dates unless they are short term problems.
Having a full 2 weeks to complete an assessment is sometimes not an option. With an admission assessment, the MDS must be completed within 14 days of admission (counting the admission date). With annuals and quarterlies, you have 14 days to complete the assessment from the ARD. With a significant change, you have 14 days to complete the assessment from the finding that a significant change has occurred.
I would pull my team together and explain the process. I would use the MDS manual as my resource and explain why dates are very important. I would also request that the administrator attend this meeting as well so that she is on the same page as the rest of the team.

My facility is having a problem with completion dates on care plans. Long term care plan goals have 3 month time frames. I think that the update must not go beyond this time frame i.e. 10-20-03 goal must be updated by 1-20-03 or earlier. The problem is MDS end dates sometimes go past this date. I have always made sure to not schedule end dates past the day the CCP needs updating but the other MDS scheduler has not. Administrator wants me to change my schedule and says that the people completing CCP just need to be aware not to update CCP beyond the 3 month time frame. I think this leaves too wide a margin for error and do not know how to convince her of this. Also, I have always given team 1 week to complete MDS, now I am being told I must give 2 weeks between start and end dates which in my mind will only result in more frequent assessments and more room for mistakes on care plan dates. Any ideas/feedback? |
