If while we are doing a full comprehensive annual assessment, we find there has been a significant change no more than two weeks before the annual, can we still do the annual, since it will provide raps etc for care planning, or do we need to do the sig change instead of the annual? thx.

I use a Significant change rather than an Annual if there is a significant change.  I think that is okay since they are both full assessments with RAPs.
