Hi Chriss,
I emailed you a few weeks ago regarding your care
conferences, your information has been helpful and I am trying to re-vamp
our process. Can you enlighten me a bit about the type of careplans that
you are using?

WE HAVE 21 MEDICARE BEDS, PRIMARILY USED FOR SHORT TERM STAYS. WE HAVE
CAREPLANS EVERY TUESDAY FOR THEM ALL. IT IS LIMITED TO 15 MINUTES EACH,
SCHEDULED BACK TO BACK. THEIR TIME STAYS THE SAME TIME EVERY WEEK. WE GIVE
A BRIEF THERAPY REPORT, STATUS REPORT OF D/C PLANNING.
THANKS ALOT!

We are a forty bed hospital based skilled unit. Average los is 12-14 days
and we run an average of five admits and discharges a day and usually keep
out unit full. We do care conferences one hour a day twice a week on Tuesday
and Thursday. Tuesday we do even rooms and Thursday we do odd rooms. This has
worked out well for us and keeps all the disciplines up on what the patient
is doing and whether their discharge goals and dates are headed in the right
way.
