It depends which chemotherapeutic agent is used.  There is a list of HCPCS
codes in the CMS transmittal on Consolidated Billing of exclusions.  The
transmittal can be downloaded from the AANAC site.  I have found that many
of the newer chemotherapeutic agents  used by Oncologists are not
exclusions.  If the agent used is not found on the list of exclusions, the
SNF is responsible for the cost of the Chemotherapeutic treatments.
