I worked for a LTC/Sub-Acute facility owned by a very large teaching hospital. Our policy was the following:
 
A visit by a Medical Resident was counted as a physician visit, Resident's have the designation of M.D., they are able to prescribe, write orders, etc. with varying degrees of supervision (first year residents - closer supervision, third year-residents - very little supervision). They do not require co-signature of supervising physician.
 
A visit by a Medical Student was not considered a Physician vist, Students do not have the M.D. designation, they do not have authority to prescribe medications or write orders without co-signature of supervising physician.
 
This summarizes our policy, seems to make sense.
