I agree with the consultant. The Rehab Coordinator (PTA) can sign the MDS, she is verifying the information in those sections. Don’t you have LPN’s sign the MDS to verify the sections they complete? She should not be doing the RAPs since that requires “assessment.” Is your therapy department doing many of your RAPs?

Hi Everyone, I am new at a facility, the Rehab Coordinator is a PTA.  She has been filling out the days and minutes and section T on the Medicare assessments; she then has a Therapist sign the MDS. The consultant says that it is okay for her (the PTA) to sign the MDS but she can not do RAPs only a therapist can.  I would appreciate any feedback as well as, location of supporting documentation.  I am feeling nervous about the consultant, not only is she not a nurse, she has never heard of AANAC or Rena Sheppard!!!  Thanks, Sonia
