Is the gangrene due to Diabetes or Peripheral Vascular Disease?
 
You must first find out the underlying condition which caused the gangrene in order to correctly assign the ICD-9-CM code.
 
Example:  for a diabetic patient, the icd-9-cm code book provides the instruction under gangrene with diabetes to code 250.7x (x= the 5th digit) AND 785.4.
 
If there is a dx (ex. atherosclerosis of extremities) indicated as the cause of gangrene, that dx would be coded to 440.24.
 
If there is NO other cause given, the cause is assumed to be diabetic in nature especially if the gangrene is present in the lower extremities (Coding Clnic, March-April, 1986)
