Depends upon your most recent assessment, RAPs and Care plan.  If this was an expected progression of an established disease process and was already care planned, then Hospice admission would not necessarily constitute a significant change.
 
However, if your previous RAPs and care plan did not reflect the anticipated declines and potential hospice involvement, then I would think sig chg would be warranted.

Do you fill out a significant change status when the resident is moved to hospice or is this considered a expected change.
