Our facility is all Medicaid or private pay. Every residents consistently RUGS out high. We are not for profit and cater to people with degenerative neuromuscular disorders, head trauma etc.
 
I do understand that supporting documentation used in conjunction with the MDS assessment must fall within the appropriate look back period.
 
I'm not using *just* the summaries, but do include them if they fell or contain information that fell within the appropriate look back period as part of my supporting documentation . What I'm asking is, can we write assessment  notes for the MDS after the ARD and state in that note that this or that was done/happened/observed etc. during the appropriate look back period or do we have to write the note before or on the ARD.  Each discipline generally writes an assessment note a day or two after the ARD, but indicating in it that this and that happened during the appropriate look back period for the item being addressed so that we can capture anything that happened up until midnight on the ARD.
 
Now, while I'm at it, is there anything wrong with consistently setting the ARD on the same day of the week. i.e. The ARD is every Sun. and care plan meetings the following Thurs.?

Some states are more stringent than the federal guidelines and do not allow coding the MDS from summaries, there must be documentation within the observation period...usually those which pay based on the MDS.

the resident's normal limits, that is documented on and addressed

assessments/ interviews etc. took place within the look back period.
Isn't that acceptable? For instance, if the ARD is Sun., 2/1/04 and I
write a note on 2/5/04, after the care plan meeting, saying that I
assessed and interviewed the resident on 1/31/ 03 and found blah blah
blah, and then indicate that the care plan meeting was held on 2/5,
isn't that ok?

How do you all document for MDS purposes?
