I can tell you that I was told by someone who should definitely know that this is a VERY big survey issue.  Several facilities have received Immediate Jeopardy tags for not having or not following a P/P for full code residents. 
 
We are looking at ours right now.

If a resident is a full code and you do not know when they took their last breathe, then you still start a code, correct?  I need this in writing please.
