Our facility has been using them for about 3 years now.  We started out using a brand which had removable hip pads.  I can't remember what problem we had with those.  We did try another brand with a hard shell for protection & it wasn't very comfortable for the residents.  We now use Posey hipsters.  They do have a pair which fits better over diapers for your incontinent residents.  There is a gel cushion for the hips which warms to the residents body temp and form fits the resident really well.These seem to hold up the best.  We usually wash them out by hand each evening and let air dry, alternating between 2 pairs per resident.  On occasion, they do go through our laundry and still hold up.
