We use the NDC code for each ingredient and list them as separate
medications.

We have a resident getting "Miracle Mouthwash" 5cc QID.  This is a compound
our pharmacy made for us.  It is a 1:1:1 ratio of Maalox, Benedryl, &
Carafate.  We do not have an NDC code for this just "Compound".  What do I
do for section U, NDC code?? Help!
