IV if to code for part A

I just receive a resident from a Veterans facility,  SNF. According, to the H&P per MD which is signed .  That this resident had IV vanco in the snf but I have no MAR to support with sign of nurse. ONly what is written in the H& P . Would you code IV med in section P this is Part A MDS. nancy
