Don't have it in front of me but something like this:
Problem: Potential for alteration in cardiac output r/t presence of cardiac pacemaker.
Then I insert specific data from pacemaker card: make, model, volts, etc along with
date of insertion, doctor's name and number
Goal is to have hearrate within normal limits with no adverse side effects from
insertion
Approaches are to monitor VS per MD orders
Advise MD of pulse rate above or below parameters
Schedule appointment for annual pacemaker check (one doc does 2 x yr)
Arrange for transportation and escort if needed -responsibility for this last one is
not nursing but SS.
